• Print the veterinary referral form and give it to your veterinary surgeon to complete and sign (some practices may already have some of these forms).
• Your Vet will fax, e mail or post the form to us.
• We will then contact you to arrange an initial assessment.
• Please read the Terms and Conditions carefully prior to your appointment.
